Application: Financial Assistance Education Program

Please fill out the application as accurately as possible, as a discrepancy will cause a delay in your application being approved and processed. If you have more than one child, please fill out a separate application for each child. Once your application has been reviewed, we will need the following, but not limited to, information for further review and processing: 

1. Proof of identification (State issued identification card)
2. Proof of income (6 months of employment stubs, or information on income you receive) 
3. Proof of child(ren)'s relationship to you (birth certificate)
4. Child's proof of attendance at school (enrollment, school information, report card)
